Zalozmy ze zachodze w ciaze, pracuje w magazynie, praca, ktora wykonuje na codzien nie jest bardzo ciezka (oprocz stania na nogach przez 8h) ale czasami zostaje wyslana na inne dzialy, gdzie musze dzwigac. Wiem, ze nie musze mowic pracowacy o ciazy do 15 tygodnia przed porodem, ale kiedy bezpiecznie moge powiedziec najwczesniej? Czy juz od samego poczatku ciazy jestem pod ochrona? Chodzi o to zeby, od poczatku nie musiala dzwigac i robic nic co mogloby zagrozic mojemy dziecku.
Pracuje w tej samej firmie na stalej umowie poltora roku, nie jestem na zadnym probnym okresie ani nic takiego.
Czy powiadamiajac oficjalnie pracodawce ze jestem w ciazy musze mu przedstawic jakis tego dowod np pismo od lekarza? Jesli tak, to kiedy najszybciej mozna takie podtwierdzenie otrzymac?
Bardzo dziekuje, za wszystkie rady.

I am writing to inform you that I am pregnant and that my baby is due on the _______________________
I understand that as I have now informed you of my pregnancy I do have the right to a personal and specific risk assessment as set out in the Management of Health and Safety at Work Regulations 1999.
I also understand that you should regularly review my initial assessment as my pregnancy progresses to take into account possible risks that may occur during the different stages of my pregnancy.
I am writing to ask you to arrange to see me as soon as possible in order to conduct a risk assessment.

A jak juz jestesmy przy listach do pracodawcy, to w necie znalazlam jeszcze przyklady listow w sprawie rozpoczecia urlopu macierzynskiego, moze sie komus przydac.

I am writing to give notice for ordinary Maternity Leave in accordance with current guidelines which states I am to tell my employer on or before the 15th week prior to your baby's due date.
As per the MAT B1, my baby’s due date is ------ and I intend to start my maternity leave on -------.
In accordance with current guidelines I can change the date I start maternity leave by giving you notice of the new date at least 28 days before the new date or the old date, whichever is earliest. If there is a good reason why that is not possible, I am required to tell you as soon as it is reasonable.
I am aware the earliest I can start getting Statutory Maternity Pay is in the 11th weeks before my baby is due.

Please note that I have been told that I will not receive a MAT B1 certificate until I am 25 weeks pregnant.
I qualify for 52 weeks maternity leave, made up of 26 weeks ordinary maternity leave and a further 26 weeks additional maternity leave. I would like to start my maternity leave (and pay if I qualify) on (data planowanego rozpoczecia urlopu). If I wish to change this date, I will give you at least 28 days notice.
If I qualify for Statutory Maternity Pay, I understand that the actual payment will be made in the same way that you would usually pay my wages, so I will be paid on the day I am usually paid on.

I would like to take my * remaining days annual leave from the **/**/**** (inclusive) and then start my Statutory Maternity leave and pay to commence from **/**/**** for a total of 39 weeks.
Please find enclosed with this letter, my original certificate of Expected Childbirth (MAT B1) confirming my Expected Due Date (EDD) of **/**/****.
I will notify you 28 days in advance, should I wish you make any changes to the above agreement.

Adacymru, wielkie dzieki, myslisz ze takie pisemko moge napisac i dac pracodawcy juz we wczesnym stadium ciazy?

Ja nic nie pisalam ,powiedzialam tylko Managerowi ze jestem w ciazy i sami zaczeli latac kolo mnie jak kolo jajka,oczywiscie powiedzieli mi ze nie moge nic dzwigac,zaproponowali tez przeniesienie na lzejsze stanowisko itp.Dla potwierdzenia zanioslam tylko ksiegowej moja ksiazke cizy a w pozniejszym terminie zaswiadczenie MATB1 i to wszystko

Witam wszystkich,
Mój macierzyński kończy się w listopadzie a chciałabym wrócić do pracy wcześniej. Wiem,ze muszę dać pracodawcy "8 weeks notice".
Szukam w necie jakiegoś wzoru i nie znalazłam, czy macie może jakieś wzory na takie podanie, badz wiecie jak je napisac?
Bardzo proszę o pomoc i z góry DZIĘKUJĘ.

Ja powiedzialam ze jestem w ciazy na samym poczatku, zeby nie martwic sie ze podnoszac ciezary, nie narobie sobie problemow z ciaza.
Zaraz potem traktowano mnie ulgowo. Dobrze na tym wyszla.
